To qualify for an Intravenous Contrast Administration Certificate you must be a New York State licensed and currently registered technologist and must complete and submit this application to the address below with:

1. A copy of a current certificate in cardiopulmonary resuscitation issued by the American Red Cross, the American Heart Association or equivalent training program.
